Death with Dignity: Why I Don't Want to Have to Starve Myself to DeathDr. Ron Pies writes an eloquent defense of why physician-assisted suicide should not be made a legal right in Massachusetts. He compares it to a doctor helping one of his patients jump from a bridge ? something most doctors would never do.

But in making this analogy, I believe we?re removing all context and logic from the decision behind wanting to end your own life because of a terminal illness. For the patient, it?s not about the act of suicide or ending their lives ? it?s about alleviating suffering from the disease and choosing one?s own way of dying with a little dignity. It?s about patient empowerment, human dignity and choice.

That?s why in the two states where it is legal for doctors to help patients with a terminal illness, it?s referred to as the Death with Dignity law.

Because the alternative takes much of the dignity out of dying in today?s modern medical system.

?Physicians have no more business helping patients kill themselves with lethal drugs than they do helping patients jump off bridges,? says Dr. Pies.

Most would agree physicians have little business helping a person jump to their death. But it appears that Dr. Pies undermines his own argument when he suggests it is perfectly okay for a physician to let his or her patient die of willful starvation and dehydration. His rationale? Death this way is solely in the patient?s hands, and isn?t as painful as we imagine it to be. He points to the scientific evidence, because there have been studies assessing patients? pain and suffering as they?re dying through starvation and dehydration. Well, no, not quite. The evidence he points to is a single study that surveyed ? not patients ? but hospice nurses.

Now while I have great respect for the work and opinions of hospice nurses, let?s not confuse their opinions with data that would be more helpful ? from the patients themselves. But there is no such data. So we don?t honestly know ? and can?t say ? whether a patient whose doctor is okay with letting them starve themselves to death is in greater or lesser pain than one whose doctor has voluntarily prescribed a medication to hasten a terminal patient?s death.

Dr. Pies seems to be splitting hairs here. His objection appears to be that patients can end their lives if done solely on their own, because physicians shouldn?t help patients along to their death ? especially with a prescription. Yet he?s okay with a patient starving themselves to death ? something no physician would ever be okay with in any other situation (such as a patient who had anorexia).?

Physicians of such starving patients don?t just leave at that point. They too actively help the patient starve themselves by alleviating the discomfort associated with starving and dehydration. Physicians do this by prescribing a sedative, a practice known as terminal sedation or palliative sedation.

It?s not that starving to death isn?t a painful process (it is) ? it?s because the patient has been prescribed drugs ? by a physician ? to make their ?natural? death less painful.

Last, Dr. Pies argues it is not a right to die with dignity at the end of our lives, in the time and manner of our choosing. But nobody is coercing physicians to comply with the proposed law in Massachusetts. Just as most physicians do not perform abortions, I suspect many physicians will also not be interested in prescribing drugs to help a person at the end of their life hasten their own death.

We need such a law not to compel physicians or mess with their moral code, but because government has determined that the people cannot be trusted to have access to certain medications. Because government has restricted our access to such drugs, it is necessary to seek access to them through the government- and guild-defined methods imposed.

If my liberty to purchase and administer such drugs wasn?t restricted in the first place, we wouldn?t need such laws. But since my liberty has been restricted, a law is needed. This law would not impose an obligation on physicians to prescribe such medications to any patient who asked, as it would be completely voluntary for physicians to participate:

(2) Participation in this chapter shall be voluntary. If a health care provider is unable or unwilling to carry out a patient?s request under this chapter, and the patient transfers his or her care to a new health care provider, the prior health care provider shall transfer, upon request, a copy of the patient?s relevant medical records to the new health care provider.

One of a physician?s primary purposes is to help alleviate suffering. Suggesting a patient starve themselves over a period of one to two weeks, while being prescribed and administered a sedative, hardly seems in the spirit of this purpose.

For me, it?s not about a physician?s rights ? it?s about a human being?s inalienable rights and having the right to choose. So I will be voting ?Yes? on Question 2 for the Death with Dignity Act in Massachusetts. Because I believe that people with a terminal illness have a right to die at a time and place of their own choosing ? with the dignity deserving of a human life.

Teens to stage concert | Toowoomba Music | Music Reviews ...

TOOWOOMBA teen Jenyu Shaw is shaking up the belief that all teenagers are game-playing couch potatoes.

The 16-year-old Concordia Lutheran College student is organising the Toowoomba Youth Charity Concert.

It is the second time he has staged the youth-focused event.

"Last year I was practising for my exam and thought it would be good to be able to play for an audience," Jenyu said.

"There were other musicians in the same boat and I also thought we could raise some money."

The inaugural concert raised more than $1600 for the group, Jubilee Ministries, to help fund programs for the disadvantaged.

This year, World Vision will benefit from the concert.

"People definitely think that teenagers just sleep and play games," he said.

"But I know a lot of people my age who have done heaps."

Jenyu's instruments of choice are the piano and the trombone.

He, along with six other performers, will present a 10-minute piece for the concert.

"There are different people performing this year," he said.

"We have people from different schools and groups.

"The whole concert has been organised by teenage kids, and on the day, the ushers and crew are all going to be teenagers from Toowoomba.

"I have been around to many of the eisteddfods and invited some of the best youth musicians from Toowoomba to play."

Mary Evelyn McKee?s Mountain Brook home was originally built for an extended family, but for the last couple of years it?s been used by empty nesters ??until recently.

Brook home for about eight years. Her three boys are grown, but one recently brought his family back to Birmingham from California.

Temporarily, the family of three are making their home with Mary Evelyn, and true to her design style, she?s making it one the family will likely want to stay in.

?This house really was built with an extended family in mind,? she said. ?So we?ve really come full circle.?

It also comes full circle with Mary Evelyn?s theme at this year?s Birmingham Botanical Gardens? Antique at The Gardens. Mary Evelyn is one of the tastemakers, and her theme will be pied-?-terre, a term frequently used in big cities to refer to a small, secondary living space ? or in the case of Mary Evelyn and part of her home, a temporary living space.

When it comes to tackling such spaces, Mary Evelyn takes the same approach as she would in a larger permanent space: It should be comfortable and inviting, combining special family pieces with beautiful design.

?I?m inspired by personal items of the people who inhabit the space,? she said. ?It has to have a soul. It has to come from the people who spend time there.?

Evidence of this philosophy is found all over Mary Evelyn?s own home.

Whether it?s in her permanent living space or her granddaughter?s room, Mary Evelyn mixes family heirlooms with a combination of modern and traditional pieces to create spaces that are inviting and, as she likes to say, ?less than perfect, and not too demanding.?

Take her granddaughter?s new room, for example.

That was a fun one for Mary Evelyn. Being the mother of three boys, she said it was fun to create a room for her 3-year-old granddaughter, Madelyn.

While it would have been easy to go with an all-over pink ? which there is plenty of in the bedroom ? Mary Evelyn added some surprising yet still feminine touches, like the wallpaper. The Nina Campbell wallpaper is covered in butterflies but has a softer palette. Mary Evelyn liked the way the pattern paired with an old rocking chair that has been in her family for five generations.

The old wooden single bed frame is also special to the family. It was Madelyn?s father?s when he was a child; now, it?s covered in hot pink bedding. Pillows in shades of pink by John Robshaw decorate the bed. Another splash of hot pink is found in the chair and chaise where Madelyn likes to sit and read.

?I?ve never had a little girl, so I might have gone overboard on the pink, but I think that?s OK,? Mary Evelyn said. ?I love the quote that ?hot pink is the navy blue of India.??

When it came to decorating the permanent living spaces in the house, the color palette is much more neutral.

The breakfast room is painted in Benjamin Moore?s Cotton Ball. Mary Evelyn designed the banquette seating with a Greek key trim along the bottom that?s paired with an antique trestle table.

?That table has been in every house we?ve had,? she said. ?It has a new life every time we move.?

Mary Evelyn found the large round mirror above the table years ago at Robert Hill Antiques. The chest on the opposite wall of the table repeats the Greek key border found on the banquette seating.

?I?ve just always been drawn to neoclassical elements,? Mary Evelyn said.

She also is drawn to art, which is displayed throughout the house.

One of her favorite paintings hangs in the gallery at the front of the house. It?s by Birmingham artist Annie Kammerer Butrus.

?It?s one of a series she?s done over the years on the Chilton County peach harvest,? Mary Evelyn said.

Mary Evelyn loves mixing storied family pieces in her home. The table, she said, has been in every home the McKees have lived in.

Mary Evelyn also loves to mix the old with the new, as she does in the downstairs powder room.

There, a modern steel bowl sink sits on top of a custom-made table. A wall of antique mirror gives the room a classic feel, and the room wouldn?t be complete without a family treasure: A trumpet vase that belonged to her husband?s great-grandmother is displayed next to the sink.

Mary Evelyn?s home is a mix of family heirlooms and classic pieces, giving the spaces a timeless feel, which is the design philosophy she?s had since starting her career in 1986.

Her success in decorating led her to open her store in Homewood about six years ago. There, just like in her home, she has a wide variety of accessories and furniture to make a home inviting yet comfortable.

Google obeys Brazilian order, blocks video

RIO?DE JANEIRO???The Brazilian unit of Google, the world's No. 1 Internet search engine, said on Thursday it had obeyed a court order to remove a video attacking a candidate in Brazilian municipal elections from its YouTube service after legal appeals were exhausted.

"We are profoundly disappointed to not have the opportunity of openly debating our arguments in the electoral justice system that the videos were legitimate manifestations of the freedom of expression and should continue (to be) available in Brazil," said Fabio Coelho, director-general of Google in Brazil in an e-mailed statement.

The legal challenges underscore broader questions about Google's responsibility for content uploaded by third parties to its websites, including an anti-Islam video that sparked a wave of protests and violence in the Muslim world.

An arrest warrant was issued for Coelho earlier this week by a court in Brazil's Mato Grosso do Sul state after Google failed to obey an order demanding removal of the video attacking a mayoral candidate.

Judges in Brazil have held executives responsible for resisting the removal of online videos in violation of a stringent 1965 Electoral Code. The law bans campaign ads that "offend the dignity or decorum" of a candidate.

Google, which says it complies with local law but fights "diligently" to protect free speech, complied with the judge's order after it ran out of appeal chances, Coelho said.

On Wednesday, Coelho was questioned by Federal Police officers over the failure to remove the video and was later released.

"Despite this, we will continue with our global campaign for liberty of expression, not just because it is a prerequisite for a free society but also because more information generally means more schools, more power, more economic opportunities and more liberty for people," he said.

'Intimidation effect'

The person or persons who posted the offending videos "ironically" removed them on their own accord and closed their YouTube account, Coelho said.

"This is just one example of the intimidation effect of such an episode for freedom of speech," he added.

Earlier this month, an electoral court in Brazil's Paraiba ordered the arrest of another senior Google executive, Edmundo Luiz Pinto Balthazar, after the company refused to take down a YouTube video mocking a mayoral candidate there.

The video clip loaded by the user "Paraiba Humor" seized on a verbal slip by a candidate in a montage remarking, "What an idiot???give him an F!"

Within days, another judge overturned the order to arrest Balthazar, writing that "Google is not the intellectual author of the video, it did not post the file and for that reason it cannot be punished for its propagation."

Broader debate

Brazil also entered a broader international debate this week about an inflammatory YouTube video depicting the Prophet Mohammad as a womanizer, fool and child abuser, when a state court in Sao Paulo ordered Google to take down the video.

World leaders have decried the video, which set off a string of violent protests in the Muslim world, including attacks on U.S. embassies in Egypt, Libya and Yemen. Several Muslim leaders called for international action to outlaw acts of blasphemy.

When President Barack Obama addressed the U.N. General Assembly on Tuesday, he repeated his condemnations of the video as "crude and disgusting" but defended the importance of free speech in the United States and throughout the world.

Ruling on a lawsuit by Brazil's National Islamic Union, Sao Paulo Judge Gilson Delgado Miranda gave Google 10 days to remove the video. In his decision, Miranda said he weighed freedom of expression against the need to protect against action that might incite religious discrimination.

Obama makes a pitch to veterans, military families

VIRGINIA BEACH, Va. (AP) ? President Barack Obama has appealed to veterans and military families in defense-focused southeast Virginia, casting himself as a president who is ending foreign entanglements and aiding returning soldiers with educational opportunities.

Obama chided rival Mitt Romney for saying, in secretly taped remarks months ago, that nearly half of Americans believe they are victims entitled to government help.

Obama said instead of victims, he sees veterans who have served "with bravery and distinction" and military families wondering if loved ones will "come home safe and sound." Obama was introduced by Sen. Jim Webb, a conservative Virginia Democrat who is a former Marine and Navy Secretary.

Romney was campaigning nearly 200 miles away in Springfield, Va. Obama won the state in 2008; it is among the most competitive of the 2012 contest.

The Spelling Bee

The Spelling Bee competition is an initiative of the English Opens Doors Program especially devoted to student?s linguistic development. The tournament, established in 2012, intends to provide students with an opportunity to build relationships among peers and compete with them in a friendly and fair atmosphere.

At the same time, this competition aims to motivate students to search and study about the English language. Further, it encourages them to spell words rapidly and correctly, demonstrating full comprehension of each word.

Why is it important to not only know how to spell the words, but also have depth of comprehension about the words?

Most researchers agree that a vocabulary of the 2000 most-frequent words is sufficient to understand approximately 80% of what is read or spoken. Therefore, knowing these words is a powerful way to increase overall comprehension of English. The consequence of this is greater enjoyment of and ability to use the English language for personal benefit and enrichment.

Public Speaking

The Public Speaking Competition, which started in 2008, is another initiative of the English Opens Doors Program (Programa Ingl?s Abre Puertas ? PIAP). It seeks to reinforce the students linguistic competence and oral communication in English. This is generally referred to as, ?communicative competence? by linguists. As an additional benefit, the competition fosters and develops the students eloquence in speaking as well as promotes their powers of persuasion, besides being an extraordinary way to have fun with English!

Osage member nominated to head Indian trust office

OKLAHOMA CITY (AP) ? A member of the Osage Nation from Norman, Okla., has been nominated by President Barack Obama as the next Special Trustee for American Indians.

The Office of Special Trustee for American Indians manages about $3.7 billion in Indian funds held in trust by the federal government. It also manages Indian leases for natural resources such as coal, oil, natural gas, timber and grazing, that generate income for the trust accounts.

Obama nominated Vincent G. Logan for the post. Logan owns The Nations Group LLC, which works with Native American tribes on asset management, investment strategies and financial education. He also is a member of the Oklahoma State University Foundation Board of Governors.

Logan received his undergraduate degree from Oklahoma State University and his law degree from the University Of Oklahoma College of Law.

The only losers in the re-branding under the Havas Worldwide banner are the four Frenchmen Bernard Roux, Jacques Seguela, Alain Cayzac and Jean-Michel Goudard, who contributed the initials of their last names to name the RSCG shop that was merged with Eurocom eons ago (1991).

They are now forever inscribed in adland?s Hall of Fame with BBDO?s George Batten, Bruce Barton, Roy Durstine and Alex Osborn; DDB?s Ned Doyle, Mac Dane and Bill Bernbach and Needham Harper?s Maurice Needham and Paul Harper. BBDO, DDB and Needham Harper were joined together in 1986 under the Omnicom moniker.

To this blogger, Havas was playing an insider game with Euro RSCG. 4 Debt Management Tips That Could Boost Your Credit Score

Many proteins exist in a state of 'disorder' and yet are functional

ScienceDaily (Sep. 20, 2012) ? If you open any biology textbook to the section on proteins, you will learn that a protein is made up of a sequence of amino acids, that the sequence determines how the chain of amino acids folds into a compact structure, and that the folded protein's structure determines its function. In other words sequence encodes structure and function derives from structure.

But the textbooks may have to be rewritten. As Rohit Pappu and two colleagues explain in a perspective published Sept. 20 in Science, a large class of proteins doesn't adhere to the structure-function paradigm. Called intrinsically disordered proteins, these proteins fail fold either in whole or in part and yet they are functional.

The following is a recent interview with Pappu, PhD, professor of biomedical engineering and director of the Center for Biological Systems Engineering at Washington University in St. Louis, who explains the latest science.

When did people realize some proteins violate the rules?

It's been about 20 years. The earliest clue was that some protein segments didn't show up in X-ray crystallography or NMR studies, the standard ways of studying protein structure.

By the 1990s people who studied how proteins interact with DNA had noticed the proteins often change shape when they interact with DNA. In the absence of DNA all the standard probes for protein structure reported back that the proteins were floppy, and yet when the protein formed a complex with DNA it had a well-defined three-dimensional structure.

How did you first come to hear about them?

By serendipity. When I was leaving Johns Hopkins University to come to Washington University in 2001 I had a meeting with Keith Dunker of the Indiana University Schools of Medicine and Informatics, one of the founding fathers of this field. It was pure chance. The meeting started awkwardly because Keith was wondering who I was and I had never heard of him. I was working on a polymer physics description of unfolded proteins, and it turned out he had just written an 80-page review paper on intrinsically disordered proteins.

"Every time you talk to people in the back alleys of protein science," he said, "they tell you their proteins are very flexible or highly dynamic, and this dynamism is important for function."

So Keith did two things. He synthesized all of the information then known about these flexible, highly disordered proteins. And, together with his colleague Vladimir Uversky, he asked if it was possible to predict which sequences would be incapable of folding autonomously. With the help of computer scientists who taught him how to look for patterns in high-dimensional spaces, he learned that 11 out of the 20 amino acids predispose sequences toward being disordered. Today there are about 20 predictors of disorder.

So when I heard this story I thought, "OK, either this is absolutely crackers or it is going to be transformative. I'm going to take a bet on transformative because I find what he's saying compelling."

So during my first two years at Washington University I started to devour the literature. I think I scared a lot of people here who weren't sure they had hired the person they thought they were hiring.

What percentage of proteins are intrinsically disordered?

It goes by kingdoms. So in bacteria and prokaryotic organisms these numbers are pretty small. They're about 5 percent of the proteome, the entire set of proteins made by an organism. But if you go to eukaryotes or multicellular organisms then the numbers get to 30 or 40 percent of the entire proteome.

But if you ask what percentage of sequences that make up the signaling proteome -- proteins that are busy passing messages to other proteins -- are intrinsically disordered, then the numbers jump up to 60 to 70 percent.

There seems to be a division of responsibilities. Structured proteins take part in catalysis and transport. Intrinsically disordered proteins are important for signaling and regulation.

Why are disordered proteins involved in signaling and regulation?

I think there are two logical reasons. One is that complexes involving intrinsically disordered proteins are short-lived and the other is that they typically bind many rather than just one molecule.

If a molecule cannot fold except in the context of a complex, then some of the energy used for folding must come from intermolecular interactions. And if the molecule has taken out an energy loan, the complex that forms is not going to be very stable or long-lived.

You're combining high specificity (because the protein will only fold when it recognizes the molecule with which it forms a complex) with low overall affinity (because the complex is not very stable).

The many-to-one interactions arise because disordered proteins typically function through short amino acid stretches instead of large protein-protein interfaces. So a single polypeptide stretch can interact with multiple targets. One motif talks to one protein, and a second motif talks to another protein, but through the chain they can communicate with each other.

That's why these molecules happen to be at hubs within networks. They're trafficking information through networks like the air traffic control tower in an airport hub. Because most of their functions are carried out by these very short motifs, they are capable of coordinating large amounts of information that are disparate in nature. You get many things happening at the same time.

What was remarkable to me about your perspective is that you emphasized functionality of these proteins. Isn't the name a bit misleading?

You're right. As we get to know them better we've thought we should have called disordered proteins, molecular rheostats. But to a physicist disorder just means thermal fluctuations are dominant, so for physicists it's an accurate description. The problem is that in the biomedical field the word disorder has been coopted for disease.

You mention several tricks these proteins have up their sleeves. One I thought was clever was modulating the local chemical environment to encourage a particular reaction.

This is a very important idea. If you're doing chemistry in a test tube, and you want to make a reaction go, you increase the concentration of the reactants: A needs to bump into B and do so often. But this is a matter of probability so you might need a gazillion molecules of A and bazillion of B to get some statistics.

But if there's a tether between A and B, they're guaranteed to bump into each other quite often. You might be able to get away with a handful of molecules instead of gazillion.

The loose tether, in effect, increases the concentration of A around B, and the tether is often a disordered region.

Another thing you mentioned was cryptic disorder: the idea that structured proteins can become disordered. That's such a backward flip.

Richard Kriwacki of St. Jude Children's Research Hospital, a co-author of this perspective, is the person who made the clearest discovery in that regard. He shows that two structured domains can come together -- this is part of the whole p53 tumor suppressor apparatus -- and in trying to commingle, they undergo an unfolding transition that exposes sites that otherwise were buried.

This is the idea of cryptic disorder: that domains, by promoting disorder in one another, reveal hidden, or cryptic, motifs or sites that now are available for function.

You mention that in the biomedical community disorder is associated with disease. Your co-author M. Madan Babu of the MRC Laboratory of Molecular Biology in Cambridge has written about this connection.

Yes. Cells make many decisions. They decide to differentiate, to die, to regenerate, or to go quiet ,and these decisions are controlled by regulatory networks. The integrators in the networks are predominantly disordered regions.

So the question is: Will mutations in those regions give rise to unwarranted cellular phenotypes and hence diseases, such as cardiovascular disorders, cancer and neurodegeneration? The answer is absolutely yes.

But what we are learning is that mutations in disordered regions don't necessarily generate a deleterious phenotype because disordered regions are fairly unconstrained compared to structured regions. So these regions are also engines of robustness.

They're more robust to mutation?

At least the one study that looked at cancer mutation would suggest that. It showed that cancer-associated mutations partition toward structured regions of proteins, not toward disordered regions.

Do you think the new awareness of disordered proteins will lead to medical breakthroughs?

Maybe. If I tell you that a disordered protein is at the hub of a network, then it stands to reason that targeting the hub with a drug gives you a ready-made way of controlling a cellular decision.

The only problem is that we don't quite know what it means to target a hub. If a protein has a very precise shape we know how to target it: it's like designing a key for a lock. But if a protein is disordered we have to understand what that means for that particular hub. We also have to be aware that anything that changes the hub will change a range of downstream processes, pathways and cellular decisions.

Nonetheless many people now are talking about these disordered proteins as druggable targets.

Together with Peter Tompa, another of the field's founders, you organized a Gordon Research Conference on disordered proteins this summer. This was a chance for the leading scientists in the field to explore their thoughts off the record. What was the consensus?

It was evident that the more you know the harder it is to draw the order/disorder demarcation. There is a continuum. In fact, many disordered regions do end up adopting structure; they just defer the adoption of structure to the appropriate context.

We wrote the Science perspective because we thought this was the opportune moment to make the point that there is probably evolutionary synergy between the structured domains and the disordered regions, and that synergy is what we really need to wrap our heads around if we are really going to get at how biology integrates signals to control processes and generate responses.

So at the end of the day what you end up with is molecular integrators and it appears that these disordered regions are the molecular integrators.
